The appointment request form sends your answers by e-mail to Estelle Derwa. They are not saved in a database or on this site. They are used only to call you back and arrange an appointment.
Retention: for as long as it takes to reply and arrange the appointment.
The appointment form is protected by Cloudflare Turnstile, which checks that a request comes from a person rather than a script. Your browser loads that check from challenges.cloudflare.com, which therefore receives your IP address. It sets no advertising cookie and does not track you across sites.
You may request access to your data, its correction or its deletion by writing to ederwa.kine@gmail.com.
Please do not put detailed medical information in the form. The “reason” field is optional and a few words are enough.

Mapping
The service-area map uses OpenStreetMap data (© OpenStreetMap contributors, ODbL). The basemap tiles are served by CARTO: when you open the “Home visits” page your browser downloads those tiles from CARTO’s servers, which therefore receive your IP address. The commune boundaries and markers are generated at build time and served from this site.
